Please return your COVID immunisation questionnaire ASAP if you have not already done so.
We are currently planning for all adult appointments. The questionnaire is attached and may be sent to UKStratCom-DMS-DPHC-MON-Grp@mod.gov.uk
Even if you have had a vaccine elsewhere or not wish to be included please send back your return. This will to allow us to update your record and, where relevant, ensure you are able to get an NHS certificate.
Please do not call the medical centre for vaccination administration but use email. This allows us to keep the telephone lines clear for patients needing other types of care.
