Sirs, Ma’ams, ladies and gentlemen.  Please use the form below to sign up for the upcoming Falklands 40 dinner night (if you have emailed expressing interest, you are still required to formally sign up via these means).  The event will open at 1900 for a 1930 start, and will end at 0100.  A transport plan will be promulgated closer to the time; details TBC, but expect pick up and drop off points at a central location on SHAPE, and within each primary Officers’ housing area for living-out Officers and their guests.  So that numbers can be determined, please indicate your requirement for transport in the form below.  Mess member plus first guest will be subsidized by the Mess (price per head between 35-40EUR). 2nd/3rd guests will not be covered by mess subsidy, but costs will be capped at 50EUR per head.

It is important to note that individual cancellations are not possible within 10 days of the event; the Mess will be financially liable for all places booked within this timeframe, regardless of attendance, due to the cancellation and change policy of the Van der Valk Hotel.  Any cancellations within 10 days for essential duty purposes only will be reviewed by the PMC.  If you sign up and then think you may need to cancel, please do so no later than 10 Apr 22 by emailing me directly (david.crawley@shape.nato.int).  Last minute additions may be possible, however for cost planning purposes, you are strongly encouraged to sign up early.
