Congratulations on your assignment to SHAPE.  SHAPE NSE will be your Admin Office on your arrival in SHAPE, they will contact you over the coming months/weeks with various bits of information and guidance to aid your move to Belgium.  You are requested to return all documentation requested in a timely manner.  The NSE can be contacted with any issues, queries or concerns along the way.

The first part of your arrivals process is…

  • Read and digest the EJSU Supportability DIN which contains vital information about welfare and medical support available in Country. Once you have read this DIN please complete the form below

  • Complete the attached Medical Screening documentation and return it to SO2 Med EJSU.

You are not to book any travel until SO2 Med has approved your Medical supportability.
