Changes to your Healthcare Service from 1 Oct 2020

Healthcare arrangements in some EJSU areas in Belgium, the Netherlands and Germany are changing slightly this autumn.  We are currently cared for in the locations below by a partnership of SSAFA and GSTT and this will soon move back to Defence Primary Heathcare in partnership with HEALIX international.

We have been able to visit and speak to many of you but if you have not had all the information you require or want to understand things more fully please email SGDPHC-O-EJSU-Med@mod.gov.uk .  If these changes affect you we hope that you have received your information and registration pack.

Ramstein

What will change?


Your healthcare will continue to be provided by host nation health professionals, but this will now
be overseen by the Central European Practice (CEP) and an industry partner, Healix International.
In most parts this will feel similar to your current care but you will no longer fall within the SHAPE
LLP practice when the contract concludes. There is no change to how you access Emergency care.

The CEP is a Defence Primary Healthcare (DPHC) Practice, based at SHAPE. We support screening
for overseas assignment; manage NHS records; provide clinical advice when a UK MoD opinion is
required and conduct occupational health management for serving personnel. There will be a CEP
healthcare coordinator within Ramstein Air Base for liaison, administration and patient support.


Primary Health Care

You will continue to access care from local primary care doctors who specialise in general practice,
paediatrics and gynaecology. We plan to continue relationships with the clinics you currently visit,
aiming for direct payment if possible. You are also free to choose an alternative licenced primary
care provider, although you may have to pay and claim the costs back.

Occupational health is a large component of primary care in the MoD and is particularly important
for aircrew. An MoD-employed doctor will continue to visit Ramstein to ensure military personnel
are fit for role and to offer discussions for complex healthcare or supportability matters.

With registration, your healthcare record will move from the SHAPE LLP practice to the CEP. The
CEP will operate as your NHS record holder and while we will not deliver your care directly, we will
manage your health record and can offer UK MoD perspective on clinical matters if required. For
servicepersons we will also manage your occupational health and any direct Defence referrals.

Secondary Health Care

If you are referred from a primary care doctor to secondary healthcare (hospital outpatient
department for example) you will call the Healix medical helpdesk for advice and approval. All calls
are answered by a team who are supported by doctors, nurses and other specialists. They are
highly experienced in international healthcare delivery and we are used to working with them.

If you are already on a treatment pathway the transition will not change this. We will seek your
consent to talk to Healix about your care and you should expect to continue on the current plan.

ultimately paid for by the MOD. As such any decisions made with you about care pathways are not
influenced by financial constraints, but are based on ensuring safe, effective and relevant care in

• A 24/7 medical helpdesk, staffed by the international healthcare team.

• Management oversight of host nation secondary healthcare referrals to ensure any

MOD clinical advice is available should it be required. Patient management is also conducted in
consultation with the CEP in instances of serious illness or when there is concern about
occupational implications. We have a good relationship already with the team.

What next?

There will be a requirement for everyone remaining in post to register with Healix
and you will soon receive instructions on how to do this. With your registration, we will transfer
your record from your SHAPE LLP practice to the CEP and ensure care pathways are transferred.

We are acutely aware that you have already undertaken a screening process and appreciate the
time that you will spend providing important details to Healix. This will ensure continuity of care
and that your personal and health information is handled legally and safely.

We appreciate that you will have questions and we will seek to answer these in various ways. You
have received Healix registration instructions and a date window. We will also send you a CEP
patient information pack for your area. Online Q&A sessions will be advertised direct and through your NSE.

Wesel

What will change?

Your healthcare will continue to be provided by host nation professionals, but this will now be
overseen by the Central European Practice (CEP) and an industry partner, Healix International. In
most parts this will feel similar to your current care but you will no longer fall within the SHAPE LLP
practice when the contract concludes. There is no change to the way you access emergency care.

The CEP is a Defence Primary Healthcare (DPHC) Practice, also based at SHAPE. We conduct
screening for overseas assignment; manage NHS records; provide clinical advice when a UK MoD
opinion is required and conduct occupational health management for of serving personnel.

Primary Health Care

For Primary Care you will continue to use the local GP of your choice. We plan to continue
relationships with the clinics you use and aim for direct payment as far as possible. You are
equally free to choose an alternative licenced Primary Care provider, although you may have to
pay and then claim the costs back via the MOD.

Occupational health is a large component of primary care in the MoD. The Senior Medical Officer
or another MoD-employed doctor will be available to ensure military personnel are fit for role and
to offer discussions for any complex healthcare or supportability matters.

With registration, your healthcare record will move from the SHAPE LLP practice to the CEP. The
CEP will operate as your NHS record holder and while we will not deliver your care directly, we will
manage your health record and can offer UK MoD perspective on clinical matters if required. For
servicepersons we will also manage your occupational health and any direct Defence referrals.

Secondary Health Care

If you are referred from a primary care doctor to secondary healthcare (hospital outpatient
department for example) you will call the Healix medical helpdesk for advice and approval. All calls
are answered by a team who are supported by doctors, nurses and other specialists. They are
highly experienced in international healthcare delivery and we are used to working with them.

If you are already on a treatment pathway the transition will not change this. We will seek your
consent to talk to Healix about your care and you should expect to continue on the current plan.

Healix contract

Healix is a medical management company not a health insurance company and your care is
ultimately paid for by the MOD. As such any decisions made with you about care pathways are not
influenced by financial constraints, but are based on ensuring safe, effective and relevant care in
line with that provided by the NHS. Once registered on the FCO healthline contract you will be offered:

• A 24/7 medical helpdesk, staffed by the international healthcare team.

• Management oversight of host nation secondary healthcare referrals to ensure any
treatment is appropriate, relevant and, where possible, in line with UK NICE guidelines.

• National screening programmes to be conducted in line with NHS standards, provided in
the overseas area where feasible.

• A referral pathway into UK secondary healthcare where appropriate and when requested.

• Comprehensive Pre-Screening to ensure medical supportability in post.

• Support for aeromedical evacuation if required.

SHAPE & Brussels Practice

What will change?

Your healthcare will broadly continue as it is now: provided by the same people and from the same facilities in SHAPE and Brussels.  The current contract is fulfilled by a partnership of SSAFA, who focus on primary and community care, and Guys and St Thomas’ (GSTT) NHS trust who facilitate hospital care. At its conclusion in September there are two main changes:

•The practice will return to Defence Primary Healthcare (DPHC) management.

•We will partner with Healix Interrnational to maintain our access to hospitals.

Primary Health Care

Regarding primary care and a return to DPHC, expect no discernible difference in the type or level of care provided. We enjoyed an excellent relationship with the SSAFA/GSTT partnership and have had military staff embedded throughout their tenure. The current and future staffing model is very similar and we are all working to ensure that each detail of the transition is covered. Your clinical records will not move and we will be able to continue with new ways of working, accelerated by COVID, such as tele-consulting.

Secondary Health Care

The second important change is that referrals for secondary care, such as hospital appointments, will be facilitated by Healix International. It is worth sharing that the EJSU medical team have daily experience of working successfully with Healix more widely across Europe and Turkey. Their role in SHAPE and Brussels will be to maintain our ability to refer to care providers in Belgium and the NHS, to deal with hospital billing and to ensure passage of translated records. If you are already on a care pathway you should expect that to continue although we may contact you to seek your consent to discuss appointment details with Healix.

Healix Contract

Healix is a medical management company not a health insurance company and your care is ultimately paid for by the MOD. As such any decisions made with you about care pathways are based on ensuring safe, effective and relevant care in line with that provided by the NHS. You will also continue to need travel health insurance for any leisure travel.

If you have experience of HEALIX from other overseas assignments your interaction with them will be slightly different – as you will remain directly cared for in a UK Defence Primary Healthcare Practice. The services they will add to the practice will include:

• A 24/7 medical helpdesk, staffed by the international healthcare team. You can obtain advice on healthcare requirements out of hours to replace the current patient helpline.

• Facilitation of hospital referrals in liaison with the SHAPE and Brussels practice team.

• Comprehensive Pre-Screening to ensure medical supportability in post.

• Communication and healthcare record sharing between hospitals and Defence Primary Healthcare.

Everyone in post will to register themselves and their families with Healix. You should have received instructions on how to do this.  We are acutely aware that you have already undertaken a screening process and appreciate the time that you will spend providing important details to ensure continuity of care and that we share information legally.
