All invitation emails for the Covid/Flu winter vaccination programme for eligible adults have now been sent out and followed up with phone calls. If you have not received an email or a phone call and you believe that you are in one of the groups below, please complete the attached consent form and return it to UKStatCom-DMS-DPHC-MON-Grp@mod.gov.uk.
- 50 and over (Turning 50 by the 31st March)
- You are identified as one of the clinical risk group
- A BMI of 40 or over
- Receiving carers allowance
If you are in one of the groups above and do not want the vaccine please indicate this on the consent form and send back to us so that our records can be updated.
